HELP NEEDED: Combine path question?

I have mastered a lot of inkscape on my own by using the wonderful tutorials available here and on blogs. I have run across one problem that I cannot seem to figure out and have not found discussed anywhere.

Let me try to explain....I am trying to do two layer images. The top layer would include inner paths that would be cut out so that the bottom layer will show through. Sometimes when I combine paths on all of the items for the top layer, in normal view I can see the bottom layer through the "openings" and sometimes the top layer is solid and I cannot see the bottom color. I have been selecting no fill on the inner objects before I combine paths. I don't know what I am doing wrong and it is weird to me that sometimes it works and sometimes it doesn't (even on the same file).

I hope I explained this well enough. Anyone have any ideas?
