not cutting right

I was making postcard type (plain rectangle) of invitations & put 4 on a sheet. (12 x 12 mat & paper)
The 1st two cut fine, the 2nd two the blade did not return to where the beginning of the cut for the rectangle started. I had my speed on low, new mat, new blade. It took a nik out of the corner & I had to use my exacto knife & ruler to make them usable. I'm new to this, any ideas what I did wrong?

If you can attach the file or a screenshot of the SCAL preview someone might be able to help you.

I didn't save the file. It was 4 each of 5 1/2" x 4 3/4 " rectangles. the 1st 2 cut good, then the blade returned to the end of the mat to start the 2nd two, it starts the cut (on the rectangle) from the lower right side,up to top, left to the left side,down to bottom , then over to the right to complete the rectangle .(at this point is where the "nick" comes in...in this "joint") does this make sense?
(in Preview the red was all solid).
I actually did 30 more of a smaller size & had the same issue. When I did this, I actually removed the 2 that I thought were the problem & cut 2 at a time & removed my mat turned it around & cut 2 on the other side. - so I only had 2 on the "screen" at this point. The 2nd two that printed were messed up also.

I used the rectangle from the shapes library from SCAL software...if that helps.
