Can someone tell me if there is a way to make a silhouette from a drawing? I found an art image (attached) but not sure how to do a silhouette of it. I've read some posts of people doing it. Can anyone explain?

Ok, this is going to show just HOW novice I really am...what does it mean when it says "paper pierced"? I've seem that a few times on some designs posted but I never understood what it meant. Does that show just how naive I am?
